Collecting urine

2)
Collect
urine and place the cup with the urine on
a flat surface. |
DO NOT URINATE/PEE FOR AT LEAST ONE HOUR BEFORE YOU COLLECT THE SPECIMEN
- Hold the cup directly in line with the urine stream.
- Collect the first part of the urine into the collection
cup, approximately 1 to 2 tablespoons or at least
one half of the cup.
- Place the cup with the urine on a flat surface.

3) Use
the dropper to place urine in the transport
tube. |
Transferring specimen
- Remove the dropper or pipette from your collection
kit.
- Squeeze the bulb on the end of the dropper or
pipette between you thumb and your first finger.
- Place the pointed tip of the dropper or pipette
into the cup filled with urine, and then release
the bulb.
- Tilt or slant the dropper or pipette so the urine
will flow towards the bulb.
- Place the pointed end of the dropper or pipette
into the tube labeled Transport Tube.
- Squeeze the bulb between you thumb and your first
finger. The urine should flow out of the dropper
or pipette into the transport tube.
- Fill the transport tube so that the urine level
is between the black lines on the label.
- Screw the cap on the transport tube as tightly
and evenly as possible.

4)
Place the transport tube in the clear plastic
biohazard bag. |
Transporting specimen to laboratory
- Place the transport tube into the clear plastic
biohazard bag along with the absorbent sheet.
- Place the plastic biohazard bag into the white
biohazard bag.
- Place this bag into the cardboard shipping box
provided in your kit.
- Keep the order form with your requisition number
and password that came in the box. You will need
to have this in order to get your results from Private
Diagnostics.
- Complete the required information on the
requisition form from the kit. You will need to
include your date of birth and the date and time
of the collection of the specimen. These steps
are very important.

An addressed pre-paid Fed Ex box has been
provided for you.
- Complete the information that is highlighted on
the requisition form and place your specimen in the
box along with the requisition form.
- Keep the order form with your password.
- Seal the box with the enclosed tape.
- Deliver this package to a Fed Ex box or
call Fed Ex at 800.463.3339 to schedule
a pick up at your location.
- Your specimen will be shipped directly to Private
Diagnostics' certified laboratory.

Place everything in the box and deliver to
Fed Ex. |

Complete the requisition form and place it
in the envelope with your specimen. |
Registering your ID and Pass Code
- Once an order is placed, an order form is generated
by Private Diagnostics, which will include both a
customer number and a unique password. To access
your lab results you will need both the customer
number and the password.
- On your order form you will find your automatically
generated customer number and unique password.
- You will also need to provide your date of birth
and the date and time of your specimen collection
on the test requisition form.
- Keep your customer number and password, located
on your order form, in a safe place.
- Included in your test kit will be instructions
on how to obtain your results through our secure
website.
Shipping
Your specimen is protected by a plastic biohazard bag, a paper biohazard
bag, and finally, a cardboard shipping box. The box
has a pre-paid Fed Ex shipping label on it all ready.
Simply deliver this package to a Fed Ex box convenient to you. Your specimen
is shipped directly to Private Diagnostics' certified
laboratory.

1)
Examine the contents of your specimen collection

The contents of your specimen collection kit
- Swab specimen collection kit containing a transport
tube and specimen collection swabs. There are two
cotton swabs included.
- Shipping biohazard bags
- Requisition form
- Order form
- A piece of tape.
- An ice pack that must be frozen for 24 hours before
shipping the specimen to the lab. More information
on this later.

Select a swab and collect your specimen. |
Collecting the specimen
Do not delay between the time when you collect the specimen
and the time when you mail the specimen. If you do, the test cannot
be performed accurately.
- Open the transport tube
- From the two swabs choose the one that seems to
fit into the lesion (sore) best. Throw the other
one away.
- The sites to collect your specimen from include
sores around your sex organs, sores around your mouth
or blisters in either area.
- Place the swab into the liquid in the transport
tube just enough to moisten the cotton tip.
- Put the top back on the transport tube and set
aside for now.
- Briskly rub the tip and side of the cotton head
of the swab on the lesion. This collects the cells
for the specimen. Please remember what the location
of the sore is as this has to be noted on the requisition
form.

Place the cotton swab in the transport tube. |
Transferring the specimen
- Take the cotton swab that you have swabbed on the
lesion with and place it into the transport tube.
It should sit in the bottom of the tube in the liquid.
- Twirl the swab and squeeze it against the side
of the tube to make sure the specimen drops into
the fluid.
- Break off the handle of the swab so that it sits ½ inch
below the top of the tube when you drop it in.
- Replace the lid on the transport tube and screw
on evenly and tightly to avoid leakage.

Replace the lid of the transport tube securely. |

Place the transport tube in the clear plastic
biohazard bag. |
Transporting specimen to laboratory
- Place the Transport Tube into the clear plastic
biohazard bag.
- Seal this bag.
- Place the plastic biohazard bag into the white
biohazard bag
- Seal this bag.
- Place this bag into the styrofoam shipping box
provided in your kit.
- The ice gel pack must be frozen for 24 hours prior
to shipping.
- You must ship this specimen immediately.
- Keep the order form with your requisition number
and password that came in your kit. You will need
to have this in order to get your results from Private
Diagnostics.
- Complete the required information on the
requisition form from the kit. You will need to
include your date of birth, the date and time of
the collection of the specimen and also the specimen
location on your body. These steps are very important.
